UC offers a comprehensive vision care benefit provided by Vision Service Plan (VSP).

The Vision Service Plan's benefits include:

• One vision examination per calendar year
• One set of corrective lenses per calendar year
• One set of frames every other calendar year (up to $130 plus 20% of any out-of-pocket costs), or
• One set of contact lenses per calendar year (up to $110 plus 15% of exam)
  
Note: if you get contact lenses, you cannot receive regular lenses until the following calendar year or frames until the second calendar year.

Eligibility
You are eligible for the Vision Service Plan if you are eligible for Full Benefits.

Enrolling in a Plan

• If you are a new employee
• If you have a newly eligible family member
• If you are a newly eligible employee

Resolving Disputes
If you are unable to resolve a dispute on your own, your location's Health Care Facilitator can help.

Terminating Enrollment

• If you leave UC employment
• If your child turns 23
